5.  How many years have you been dedicated to cutlery in general? Are you a full-time or part-time maker? If you are
a full-time maker, what made you decide to go full-time?  What problems do you face as a maker? (For full-time
makers, you may be required to provide proof of being a full-time maker.)
6.  How do you feel you have contributed to the growth of the cutlery field?  What do you see for the future of
knife-making and what is your part in this future?
7.  What traditional methods and/or processes and materials do you use in the creation of your cutlery (metals,
handle materials, carving, engraving, coloring, texturing etc)?

10.  If you were to have the opportunity to create any knife without regard to budget and/or anticipation of sale, what

11.  If you are chosen as a recipient of the award, how will you bring innovation and advancement to the custom
cutlery field?
Please provide electronic files of good quality photos (approx. 819x1024 professional quality recommended) of three
of your favorite self-made knives which you consider your most substantial knives to date.  Supply a brief explanation
as to why for each knife.  These photos must be submitted as ad image tag e.g.  the same as if you were posting  a
photo to a forum.  Below is a sample of an image tag.  These are usually generated in your photo hosting program
such as photobucket etc.

Applicant:  As a current member of the CKCA, I promise that the work I submit will be 100% mine and no outsourcing
will be done.  Also, I agree and am willing to be photographed/videoed during the year as I work on my knife which will
be viewed at the conclusion, after all judging is complete.  I agree that I am responsible for reporting any monies
given to me to the IRS as reportable income. I agree to provide my social security number if chosen as one of the
three project participants.  The CKCA is not responsible for the sale of my knife nor will they receive any payment
thereof.
